Campaign Coordinator Checklist

 Some companies use United Way of Charles County's pledge forms. Others use their own company forms. In either case, you, as your company's Campaign Coordinator, can help us process your employees' pledges accurately. As you review your employees' pledge forms, check for the following:

Has your employee given us a full legible name, home mailing address and daytime phone number?
  •  
Have they told us the total annual amount of their gift? (If they write down $5, we don't know if they mean $1 per pay period, $5 per week, or $5 per year.)

If giving through payroll deduction, have they told us how much they want withheld each pay period and how often they are paid?
  •  
If pledge form requires authorization signature for payroll deduction, is the form signed?
  •  
If they say they are attaching cash or check, is it attached? If it's a check, is it signed?
  •  
If they are giving by credit card (Visa or Master Card, American Express only), have they clearly written the 16-digit account number plus the 3-digit v-code? (The v-code is the 3-digit code above the signature space on the back of the card.) Have they signed, giving us authorization to process payment?

If, together with another immediate family member, they are giving $500 or more in a single year, have they filled in the name and company of the other person?
  •  
Has the employee indicated how he/she wants the gift distributed? If not, it will be placed into our Community Impact Fund.  United Way of Charles County only accepts donations to the agencies and initiatives listed in our brochure.

If you are using United Way of Charles County's pledge form, send the white copies back to United Way of Charles County. The yellow copies go to your payroll department (payroll deduction only; for cash contributions this copy can be destroyed), and the pink copies go to the contributors.

If you are using your own company pledge forms, please send us copies for our records, so we can track the payment process and inquire if the pledges are not reaching us.
